Benefits from National Health Insurance

Medical and dental checkups

We organize the following health checkup program for individuals who are members of the NHI in Tokushima City. Please take advantage of this program for your health.

Specific Health Checkup (Tokutei Kenko Shinsa)

(Coupon for the checkup is mailed in late June to those who are eligible.)

How to
apply

During the application period (The dates will be announced on the City’s publicity paper “Koho Tokushima”). Please come to Counter No.7, Health Insurance & Pension Division (Hoken Nenkin-ka) on the 1st floor of City Hall, with your NHI card.

Make a reservation for the dentist of your choice by phone or online etc. (Choose from the members of Tokushima City Dentists’ Association.)
Take your NHI card with you to your appointment.
Appointment is available from June 1 until July 15.

Treatments that NHI does NOT cover

NHI does not cover the medical treatments specified below, and you are required to bear all expenses for them.

  • Treatments and medication that health insurance programs do not cover
  • Additional expenses for hospitalization in a room with fewer beds
  • Personal medical checkup
  • Preventive injection
  • Cosmetic surgery
  • Orthodontia
  • Injuries and diseases that workmen’s accident compensation insurance can cover
  • Injuries and diseases caused by criminal acts or fighting

Report of injuries and diseases caused by a third party

As a rule, medical expenses for the treatment of injuries and diseases caused by a third party such as a traffic accident should be paid by the guilty party. You will be required to submit a report to us when you receive treatment from a doctor. If you are involved in a traffic accident, report it to the police and ask them to give you a Traffic Accident Certificate. Then come to our office at the Insurance & Pension Division (Hoken Nenkin-ka.)

Required materials:

  1. NHI card
  2. The Individual Number (My Number) card or notification card of the person who received injury
  3. ID of the person completing the application (e.g. Residence card, Special permanent resident certificate, Driver’s license, etc.)
  4. Traffic Accident Certificate issued by the police
  5. Seal (inkan)

If you undergo expensive medical treatment

In the event that you must undergo expensive medical treatment (both outpatient and inpatient procedures), you can apply for the coverage limit option. With a coverage limit card (Gendogaku Nintei-sho), your out of pocket medical cost at each hospital will have a specified ceiling. The coverage limit card can be obtained by application. For individuals who are 70 years of age or older and whose household income category falls on “actively working (high) income earner (geneki-nami shotoku III)” or “general (ippan),” a receipt certificate for the elderly (Koreisha Jukyu-sho) can be used to apply a coverage limit instead of the coverage limit card.

Requirements:

  1. Health Insurance Certificate
  2. The Individual Number (My Number) card or notification card of the head of household and the person receiving the treatment
  3. ID of the person completing the application (e.g. Residence card, Special permanent resident certificate, Driver’s license, etc.)

Lump-sum birth allowance

Funeral allowance

When those who are insured by the National health insurance of Tokushima City pass away, a funeral allowance of 20,000 yen will be provided to the person who conducts the funeral (moshu).

Required materials for the application:

  1. Seal (inkan)
  2. Document that proves the applicant conducted the funeral
  3. The Individual Number (My Number) card or notification card of the deceased person
  4. ID of the person completing the application (e.g. Residence card, Special permanent resident certificate, Driver’s license, etc.), and insurance card
